Step 1: Call for an appointment at 330-799-8204
Step 2: Complete a registration packet
Step 3: Parent/Guardian must be present and bring the following documents to the appointment.

_____ Birth Certificate
_____ Current Immunization Record - Doctor can fax to 330-799-6373
_____ Parent/Guardian’s drivers license/State ID
_____ Three different, current proofs of residency
_____ *If applicable, proof of custody; most recent court order, foster placed paperwork, etc.

Examples of Three Different Current Proofs of Residency:
*City of Campbell occupancy permit.
*City of Campbell tax bill.
*One current utility bill. (electric, cable/satellite, water, or gas)
*A paycheck (s)/stub showing employer and place of residence.
*Public Assistance check stub.
*Current bank statement or major credit card bill.
*Mortgage Account -proof of home ownership, property tax bill, payment or deed.
*Lease agreement.
*Ohio auto title or auto mail-in Renewal Notice.
*Professional License - issued by an Ohio government agency.
*Concealed Carry Weapons Permit.
*Social Security Administration document.
*Jobs and Family Services administration document.
*Child Support documents.

Documents MUST show parent/guardian’s full name, address, and date within last 30 days
